We thank you for your question. As far as we are aware, there are no ISPD guidelines/recommendations detailing the initial fill volumes for a patient new to PD who has had the PD catheter inserted over two weeks before and is now ready to start PD (new PD start). We conducted a small survey of members of our ISPD Nursing Committee (from Australia, Brazil, Canada, the U.K. and the U.S.A.) detailing the practice in home PD units they are familiar with, when the PD catheter has been in place for at least two weeks. We found that the usual practice is quite varied: one started with 500 cc fill, as described above; one with 1 L fills on day 1, 1.5 L day 2 and then increase to 2 L; one used a range between 1200―1800 ml fill to start; one would start with a 2 L fill; and one, for incremental PD, would start 1 or 2 CAPD exchanges with 1.5 L fill. However, all would adjust for the individual patient’s size, especially for smaller patients, would consider starting with a lower initial fill volume. Moreover, all would monitor the patient’s PD catheter exit-site dressing for moistness/wetness indicating a possible PD catheter exit-site leak.
Now, if the patient has temporarily stopped PD because of, for example, a PD catheter exit-site leak or hernia repair, and needs to restart PD, there are some specific ISPD recommendations/publications in Peritoneal Dialysis International (1,2).
