icodextrin is supposed to be prescribed once daily in agreement with current regulatory agencies worldwide. The reason for this is the risk of maltose (a metabolite of icodextrin) accumulation. However, preliminary data (f.i. Ballout A et al Perit Dial Int) suggest that a double dose of icodextrin/day together with some glucose bags is safe and efficient, and allows to get a better UF in long-term PD patients. A caution concerns the serum Na determination (interference in its dosage with maltose interaction). The use of glucose (and the residual renal function if any) allows the elimination of icodextrin metabolites and avoid their accumulation.
