Dysequilibrium syndrome rarely occurs in somebody who is receiving dialysis. But on conversion from PD to HD, many patients are not used to the hemodynamic stress of HD. For those conversion due to peritonitis, the sepsis, poor eating can all lead to hemodynamic instability. I usually start the first HD session shorter, like 2-3 hours, and then gradually increase the duration of treatment each time if they can tolerate.
