What is your experience in PD after Pancreas Transplantation?

1 answer

we have a busy kidney and pancreas transplant unit and at the same time a PD program so I will give you an answer based on our own experience:
1) we take out the PD catheter at the moment of surgery.
2) we use HD either by a native AVF or a new HD catheter in case of ARF post tx
3) as a result of 1 and 2 we do not perform PD in kidney pancreas pts but that does not mean that PD cannot be applied in extreme situations where no possible vascular access is available. We must be aware that pancreas tx is mostly done intraperitoneal when using an intestinal anastomosis so the risk of peritonitis is high.
Although we have no experience, after the first 2 months of a successful pancreas transplant I can imagine that the use of PD is really possible
Also, in the acute phase of an “extraperitoneal” pancreas, as was done some years ago ( segmental pancreas with neoprene, technique from france), PD could be easily applied
