I have a patient with ESRD under peritoneal dialysis, and she was admitted for scheduled surgery for ventral hernia. I was wondering if peritoneal dialysis should be kept or should shift to hemodialysis after surgery!

Almost every patient such as this can be kept on PD. I generally skip PD entirely for the first 2 nights postoperatively and then have them do NIPD (cycler) while supine with low volumes. In the immediate postoperative period, the patient is most likely not working, so s/he can do a larger number of exchanges with low volumes over a longer period of time. The volume can be ramped up gradually with the number of exchanges and time gradually decreasing. The exception might be the very large muscular (not just fat) patient with little or no residual kidney function. But I would still try PD first.
