If my Pt is on CCPD in the hospital, and I need to dwell Fortaz during the day, does the dose need to be different? He has been on 1 gram during the day. But at home he is CAPD. We cycle him at night during hospital stays and need to know how to dose while in house.

there are tw ways to dose cephalosporins in PD patients. The continuous dosing schedule involves adding cephalosporins in each bag – whether the patient is on CAPD or APD. The way to do this continuous dosing is 500 mg/L in the first bag nd 125 mg/L since then. Many units, however, use intermittent dosing. In this case, 1 gm is added to one bag during a 24-hour period. You want the antibiotic to dwell for at least 6 hours. For APD patients, it is best to dwell it in the day dwell – in patients who do a mid-dayexchange, that is the exchange to use the antibiotic. For the CAPD patients, it is best to add antibiotic in the overnight dwell.
