I have a patient whose job is very physical. What would any of you recommend to help prevent hernia development or membrane rupture?

It depends on the patient’s body size and PD regimen. APD would obviously be the best choice. If possible, keep the day fill volume to 1 liter, and make sure they are well healed and have been on their full regimen for at least 4 weeks before returning to work. You might have to compensate with higher night fill volumes. Make sure to take the membrane transport into consideration. It is really helpful to develop the prescription with one of the computerized programs so you can try different scenarios to see which one predicts adequate dialysis. If the job requires heavy lifting, remind them about using good body mechanics, and limiting to 20-25 lbs.
