i have a patient who is newly stated on Pd for 2 months .He devloped leaking after an exit site infection , no tunnel infection & no peritonitis . The exit site infection resolved , we held the pd for few days , tried to decrease the fill volume & still leaking .shull we removed the PD & reinsert a new cathter what is the ispd guidlines in this case ? thank you

in our experience, if the PD catheter still leaking after a month, it means that it is not properly implanted and put the patient at high risk of infection. She already had one episode of ESI, therefore keeping the catheter will put her to further risk of peritonitis, thus jeopardising the PD program. It is advisable to change the catheter (removing it) and reinsert it on the same setting provided no infection is established. We do that in our centre. So far no ISPD guideline stated on this, and the practice depends on individual centre.
