Alternative pathology should be excluded by endoscopy and abdominal imaging (eg CT scan). Assuming that no cause is found, I have had a patients who experienced intractable singultus (hiccups) as a result of diaphragmatic irritation by the conventional PD dialysate (the hiccups tended to be worse in the first 30 minutes of an exchange). In this particular case, conversion to a neutral pH, bicarbonate-buffered dialysate was effective.
