In answer to the first question PD clarity can be checked on CAPD by looking through the window of the bag, use a peice of paper underneath so writing can be seen. On APD sample bags can be used to check fluid clarity 2-3 times per week or check the drain bags the same as above. For exit site practices refer to ISPD guidelines on exit site practices, for chronic care alternate days is sufficient of if patient having daily showers then daily dressings.The guidelines give details of all practices and can be used in your policy as a reference.
