How successful is Sx for scrotal edema (possible patent processus vaginalis) in a patient on PD? Is their recurrence after Sx? Is this a more significant problem in this patient who as ADPKD with moderately enlarged kidneys? Should Sx correction be attempted at all or patient just switched to hemodialysis?

Surery is highly successful in this setting with a low recurrence rate (even in the setting of ADPKD, which does have an increased relative risk of failure, although the absolute risk is still small). Whether surgical correction should be attempted depends on the individual patient circumstances, but in general, yes, surgical correction should be attempted.
