Hello, I have a patient with Iron overload due to Haemachromatosis who is unsuitable for venesection due to renal anaemia. i would like to know if you have experience of IP chelation agents and protocols for same. Any literature I can find is old. IS there a newer better way to do this. Thank you.

IP Desferosamine (Desferol, DFO) once a day in the overnight dwell, has been used very well in the old days to chelate aluminium in old days with lots of aluminium toxicity. The affinitiy of DFO to Fe is even higher than for aluminium, so it will be effective to remove Fe overload. You may use it twice weekly and monitor the ferritin level. With prolonged use, the major side effect is mucomycomosis which can be fatal. Patients should be warned and monitored closely.
